    Inner ear infections usually develop when a virus attacks the inner ear or the nerve branches that run through it. Most inner ear infections are viral. In some cases, the infection directly affects the labyrinth or vestibulocochlear nerve. In others, a virus like measles, influenza, or mononucleosis affects the entire body including the inner ear.

    Bacterial infections are less common. Most of the time, a bacterial inner ear infection happens because of a chronic middle ear infection. Toxins from this infection invade the inner ear structures and nerve. In rarer cases, the bacteria themselves invade the labyrinth from the middle ear or the cerebrospinal fluid that flows in or around the brain.

    Injecting steroids into the middle ear works just as well as taking them orally when it comes to restoring hearing for sudden deafness patients. This finding, the result of a large clinical trial comparing the therapies, will help doctors choose the best treatment for patients with this condition.

    Sudden deafness, also called sudden sensorineural hearing loss, is an emergency medical condition that affects several thousand people annually, usually between the ages of 40 and 60.     If you have a bacterial inner ear infection, you may need an antibiotic. However, most inner ear infections are caused by viruses, not bacteria. Antibiotics arent effective against viral infections. For viral infections, your doctor will suggest treatment options to control your inflammation and help with your symptoms.

    What treatments can help?

    If you have a viral inner ear infection, your doctor may prescribe a steroid treatment to reduce inflammation or antiviral medication to attack the virus.

Well, I think most people are aware of the typical signs of an infection, like swelling, redness. Like if your throats inflamed, your doctor can look and see how red and swollen it is. And so, you know those typical signs. But when prednisone is there, its masking some of those signs. They might not actually even show up. Like you might not get the redness. You might not get the swelling. Theoretically, you do still get a fever. Hopefully, if the rest of your immune systems intact. So hopefully, the monocyte cells are still able to say, Hey, we need to attack this and send the signal with a fever. And so, look for a fever above 100.5.

    What Is The Inner Ear

    Your inner ear plays an important role in your hearing and balance. Its primary function is to receive sound waves that come in through the outer ear and go through the middle ear, where the sound makes your eardrum vibrate. These vibrations travel to the inner ear, which transmits them to the brain as nerve impulses.

    Your inner ear is a labyrinth of fluid-filled tubes and sacs. It has three main parts:

    • The cochlea, a shell-shaped structure that sends sound impulses to the brain
    • The semicircular canals, which use your heads movement to sense your position in space
    • The vestibule, which is your primary balance system

    Messages run through this system to the brain by way of the vestibulocochlear nerve. This nerve has two branches. One is responsible for hearing and one for balance. An inner ear infection happens when this nerve or the structures around it get irritated or inflamed.

    The Eustachian tube bypass procedure is designed as a long-term solution for patients who have chronic Eustachian tube dysfunction and recurrent middle ear infections. It involves a special tube that ventilates the middle ear through a tiny tunnel in the ear canal.

    How do I know if Im a candidate for the bypass procedure?

    The procedure is offered to patients who have already needed several myringotomy tubes in the past and continue to suffer from Eustachian tube dysfunction. Typically, the patients have had ETD for many years. The surgery is especially offered to patients who no longer want to have repeated myringotomy tubes.

    There are patients with poor middle ear ventilation for whom traditional tubes are not advisable. In these patients, poor ventilation may result in the eardrum being pulled inwards , making it difficult or inadvisable to place a regular tube. The bypass tube allows the eardrum to heal, giving the patient more options in the future.

    What is the difference between the tube used in bypass procedure and a regular myringotomy tube?

    The Eustachian tube bypass procedure uses a specialized tube that ventilates the middle ear through a small tunnel in the bony ear canal. It does not require any incision on the ear drum, so it minimizes any hearing damage to the ear.     Ear Wicks And Dressings

    If the ear canal skin is very swollen due to a severe infection, it may be impossible to apply antibiotic/anti-inflammatory ears drops as access is restricted. Optimum treatment involves inserting a small cellulose wick , about 1.5cm long, using the operating microscope and micro forceps. The wick remains in place for a few days and facilitates local drug absorption.

    The outer part of the wick is just visible at the entrance to the ear canal and when moistened with drops it enlarges to the size of the swollen canal. Drops are subsequently instilled at the prescribed frequency, usually three times per day. As the wick is in close contact with the skin lining most of the length of the ear canal, the drug is delivered locally to all the required area. The wick is usually removed after a few days, when the swelling has resolved, and treatment can be continued by applying the ear drops in the normal manner.

    Steroid Pills Often Fail Against Bronchitis

    Research shows that, like antibiotics, these drugs won’t reduce duration or severity of symptoms

    WEDNESDAY, Aug. 23, 2017 — Doctors sometimes prescribe a steroid for patients with bronchitis or other troublesome chest infections, but a new British study says the approach isn’t warranted.

    “Our study does not support the continued use of steroids as they do not have a clinically useful effect on symptom duration or severity,” said study lead researcher Dr. Alastair Hay, who teaches primary care medicine at the University of Bristol.

    “We would not recommend their use for this group of patients,” Hay said in a university news release.

    For the study, Hay’s team tracked outcomes for nearly 400 adults with acute lower respiratory tract infections. The participants, who did not have asthma, were split into two groups. Half of the patients received 40 milligram pills containing an oral steroid for five days, while the other half received an identical inactive placebo for the same length of time.

    None of the patients had pneumonia or bacterial infections that required antibiotics, the researchers noted.

    The study, published Aug. 22 in the Journal of the American Medical Association, found the patients on steroids had no reductions in severity or duration of their cough or other symptoms, compared to those who took the placebo. That suggests that the steroids weren’t effective in treating the chest infections, Hay’s team said.
