    Adverse Effects Of Systemic Gcs

    Although GCS play a key role in the treatment of various inflammatory disorders, including chronic upper airway disease, a quite extensive range of potential AEs is well-described in literature and the chance to develop these effects seems to increase with higher dose and longer duration of treatment .

Maybe Ive been living under a rock, but I was surprised to learn that doctors often prescribe oral or injected corticosteroids for acute respiratory tract infections like sore throat, sinusitis, bronchitis and the common cold even though evidence of benefit is sorely lacking and risks of the drugs are widely known.

    Yet a recent analysis of nearly 10 million outpatient medical visits in the United States showed that nearly 12 percent of patients with acute respiratory infections were prescribed oral or injected steroids, and this dubious practice is on the rise. The analysis found that prescriptions for steroids like prednisone to treat acute respiratory ailments nearly doubled from 2007 to 2016.

    Although steroids can be invaluable, even lifesaving, medications often vital to treating asthma, autoimmune conditions, and chronic pulmonary disease and preventing transplant rejection, their misuse can result in a treatment that is worse than the disease.

    What Are The Best Home Remedies For Sinus Infections

    There are a few simple home remedies that can be effective for relieving your pain and helping you clear your sinuses. To avoid using medications, try a warm compress on the nose and face, and flush nasal passages with a warm saline irrigation, says Dr. Elmore.

    Even though we often say we have a sinus infection even if its just inflammation or an allergic response, there are symptoms of an actual infection that may be treatable with antibiotics. Nasal congestion and pain under the eyes or around the temples are, of course, main symptoms, but others include the loss of the sense of smell, green nasal discharge, mucus dripping down your throat, cough, fever, fatigue, sore throat, and even bad breath.

    The evidence says NO to oral steroids for acute uncomplicated sinusitis. Oral steroids may help moderately with symptom relief for a few days but the 30 day outcome is the same. Serious adverse events are uncommon however so it is still common practice many places.

    In general, uncomplicated acute sinusitis is treated conservatively for 7-10days, assuming a self-resolving viral infection is to blame. Oral decongestants, NSAIDs, saline nasal rinse, nasal steroids all are helpful. When symptoms are persisting longer than expected for a viral illness then generally we turn to an appropriate antibiotic class and duration. Relapsing infections should consider early sinus CT imaging to confirm diagnosis.

    How Do You Diagnose Chronic Sinusitis

    The presence of two or more of the listed symptoms for at least three months raises the suspicion of a chronic sinus infection.

    In such cases, we will evaluate you to confirm the diagnosis. This involves applying pressure on your sinuses to elicit tenderness. If the tenderness is positive, your sinuses are likely to be inflamed. We will then take a peek into your nose using a small flexible scope, and check for nasal polyps, pus-filled discharge, and deviated septum.

    We may also recommend nasal endoscopy. This is an office procedure that enables us to view the interior of your nose and sinus passages. Its done with an instrument called an endoscope, which is a thin, flexible tube with a tiny camera and a light. We will pass this scope into your nose and sinuses to look inside. Nasal endoscopy allows us to detect any swelling and polyps, as well as collect discharge from the infected area. This can help spot the cause of your infection and whats the best way to treat it.

    Where Does This Leave Us

    What all these reviews have shown is that more and better research in this area is clearly needed. Meanwhile there is certainly some benefit to be had from using saline irrigations and intranasal corticosteroids. For patients seen by specialist doctors where polyps are seen in the nose, steroid tablets may also have a place in treatment. In cases where no polyps are seen, the case for antibiotics is less clear and should be discussed carefully between the doctor and patient.
