But Sometimes Antibiotics For Sinus Infections Are Needed
So how does one judge when it is appropriate to prescribe antibiotics for a sinus infection? There are several sets of official guidelines, which are all similar. When a patient has thick, colorful nasal discharge and/or facial pressure or pain for at least 10 days, they meet criteria for antibiotic treatment. If a patient has had those symptoms, but the symptoms seemed to start improving and then got worse again, then even if its been less than 10 days, they meet criteria for antibiotic treatment.
The authors, however, also suggest that doctors discuss watchful waiting with patients and explain that most sinus infections clear up on their own in one to two weeks, and its a safe option to hold off on antibiotics. The symptoms can then be treated with a cocktail of over-the-counter medications and supportive care, like nasal saline irrigation, nasal steroid sprays, decongestants, and pain medications.
Of course, many patients expect and demand antibiotics for sinus infections, and even those who are open to watchful waiting may hear about the rare but possible complications of things like, oh, brain abscess, and opt to treat.
In the case of my patient above, she met criteria for treatment. She weighed the watchful waiting option against the potential risks of antibiotics for her sinus infection, and chose the prescription. I can tell you from very close follow-up that she improved quickly, though in truth, we will never really know if she would have gotten better anyway.
Some Steps You Can Take
Whether your sinus infection turns out to be viral or bacterial, you can help to ease your symptoms early on with supportive sinus care:
If your symptoms arent improving after one week, its important to see your doctor. If a bacterial infection is suspected, youll probably need to take an antibiotic to clear up the infection and prevent further complications.
If your infections occur more frequently, and your doctor really wants to establish if they are bacterial or viral, your Otolaryngologist or ear, nose and throat doctor can sample the snot from your nose when youre infected and send it to a laboratory to know for sure.
Note: Antibiotics wont help a viral infection, and taking an antibiotic unnecessarily can do more harm than good. You risk possible side effects and increase your chances of developing antibiotic resistance, which can make future infections harder to treat, says Dr. Sindwani. So its important to wait and see how long your symptoms last.

Sinusitis symptoms that last for more than 12 weeks could be chronic sinusitis. In addition to frequent head colds, your risk for chronic sinusitis also goes up if you have allergies. Ways To Recognize Serious Signs Of Sinus Infections
#1: Duration
The length of the infection is an important determinant of the seriousness of the infection.
I usually consider most infections less than 3 weeks to be viral or inflammation related to congestion. At this point, the best treatment is usually medications that decrease the congestion and inflammation. This in turn will alleviate the symptoms and ultimately cure the illness.
When the illness continues beyond 3 weeks, bacterial infection can begin to develop. Though antibiotics can be considered at this point, other treatments may still be the best answer if they have not yet been given a try.
#2: Mucous Color
I will dispel a myth right here and now. Yellowish/greenish mucous does not necessarily mean the infection is bacterial.
Viruses can cause the same color mucous. The reason for the mucous is generally not the actual bacteria or virus, but the bodys immune response to the intruder.
So dont worry just because you see a colored mucous when you blow your nose. This will also improve as the infection abates.
#3: Sinus Pain
Sinus pain can occur anytime throughout a sinus infection. This is normal and means there is inflammation in the sinuses, as we discussed previously.
However, severe pain, redness over the skin, hardened skin over the sinuses, or even a severe headache are not generally normal and can indicate a bacterial infection.
#4: Fever
A fever can be caused by both viruses and bacteria. So how do you differentiate between the two?

How To Tell If These Remedies Are Not Working
You will know if these remedies are effective because you will begin to feel better and your sinuses will be less congested.
However, unlike with antibiotics where symptoms start to diminish quickly, natural remedies typically take longer to work. So you should continue to do these remedies regularly for at least a week or two before determining if they are working.
Saline Spray And Sinus Rinses
Using a saline solution, rinse your nose and sinuses with a neti pot or other irrigation system.
You can buy saline solution over the counter or make your own at home with distilled water and saline packets. Saline spray also comes in small bottles that you can spray directly into your nose.
Sinus rinses like these work by flushing mucus out of the nose and sinuses to keep things flowing freely.

What Tests Diagnose The Cause Of Sinus Infections And Sinusitis
Sinus infection is most often diagnosed based on the history and examination of a doctor. Because plain X-ray studies of the sinuses may be misleading and procedures such as CT and MRI scans, which are much more sensitive in their ability to diagnose a sinus infection, are so expensive and not available in most doctors offices, most sinus infections are initially diagnosed and treated based on clinical findings on examination. These physical findings may include:
- redness and swelling of the nasal passages,
- purulent drainage from the nasal passages ,
- tenderness to percussion over the cheeks or forehead region of the sinuses, and
- swelling about the eyes and cheeks.
Occasionally, nasal secretions are examined for secreted cells that may help differentiate between infectious and allergic sinusitis. Infectious sinusitis may show specialized cells of infection while allergic sinusitis may show specialized white blood cells of allergy . Physicians prescribe antibiotics if the bacterial infection is suspected. Antibiotics are not effective against viral infections many physicians then treat the symptoms.
In addition, both rigid and flexible endoscopy has been used to obtain diagnostic material from sinuses. These procedures are usually done by an otolaryngologist under topical and local anesthesia. Occasionally, there may be a need to sedate the patient. Some investigators suggest that endoscopy specimens are comparable to those obtained by needle puncture.
Types Of Antibiotics For Pneumonia
There are multiple types of antibiotics that work in slightly different ways. Some are more commonly used to treat pneumonia than others based on things like:
- The bacteria causing infection
- The severity of the infection
- If youre in a patient group at greatest risk from pneumonia

When To See A Doctor
Remember, acute sinusitis can last for several weeks. Give your body a chance to fight it off, using self-care techniques and getting plenty of rest.
If your symptoms start to improveeven if it seems to take a whilethen let it be. Be patient. Your body is slowly, but surely, doing what it needs to do to fight the infection.
However, if your sinusitis symptoms get worse despite proper self-care if theyre accompanied by high fever, a lot of facial swelling, and green or foul-smelling nasal discharge and if these progressively worsening symptoms persist for longer than 10 days, its time to see your doctor.
In these cases, an antibiotic may be warranted because the infection has gotten out of control.
On a final note, be sure to take probiotic supplements while on antibiotics. While important and often lifesaving, antibiotics have a downsidethey also kill beneficial bacteria along with the pathogenic.

How To Prevent Sinusitis
- Drink a lot, whether it is water or juice. More than two liters a day, especially if the symptoms continue.
- Avoid dust or smoke pollutionbecause this further irritates the nasal cavity.
- I prefer humid places, and To wet the room, you can place water or use special equipment.
- Wet your nose with compresses of hot water. Showers are also recommended because the steam helps release the nasal sinuses.
- Do not use too many inhalers because, in the long run, they will be counterproductive.
- Clean your nose well. You can find some products containing water and salt in the pharmacy.

When Antibiotics Are In Order
The main reason to prescribe antibiotics is for patient comfort, Dr. Sindwani says. The medical field used to be more convinced than it is today than untreated sinusitis would inevitably become a chronic issue, he says.
We dont think that way as much, he says. We dont know that an untreated acute sinusitis, if left untreated, will grumble along and cause people to have a chronic sinus infection.
Some people think thats two separate things, with chronic sinusitis more likely due to underlying issues like allergies or immune problems.

Rare Cases Can Turn Serious
Antibiotics also can help ward off rare but potentially dangerous complications that arise when a sinus infection spreads to the eyes or brain, Dr. Sindwani says.
Complications around the eyes are the more common of the two. These complications can cause redness, swelling around the eyes and reduced vision, and even lead to blindness in a severe form known as cavernous sinus thrombosis. Serious cases are immediately treated with IV antibiotics. Patients are usually admitted to the hospital for a CT scan to see if fluid needs to be drained, Dr. Sindwani says.
Also in rare cases, sinus infections in the rear center of ones head can spread into the brain. This can lead to life-threatening conditions like meningitis or brain abscess, Dr. Sindwani says.
Before antibiotics, people would die from sinusitis, he says. But he emphasizes that such complications are unlikely. In most cases, the bacterial infection goes away, especially if you dont have underlying medical problems.
Its important to monitor your symptoms if you suspect a sinus infection. If the condition lingers or worsens, call your doctor.
