What Is Sinus Infection

As the name suggests, sinus infection is infection of air-filled cavities in the skull called sinuses. The infection causes inflammation, which is starting point for other problems to occur.

There are several types of sinuses. The name of each type is named according to where its located.

  • Sinuses behind the forehead called frontal sinuses.
  • Behind the nose .
  • Behind the eyes .
  • And behind the cheekbones .
  • These air-filled cavities are lined with mucus membranes. Mucus is produced to help protect your nasal passages and sinuses. It will naturally drain into the nose.

The inflammation of sinus can cause obstruction and prevent your mucus from draining as well as usual. As a result, there will be more buildups of mucus in your sinus cavity and this can worsen the problem.

Based on how long it lasts, it is classified into two main categories acute and chronic. Acute means it lasts less than a month . Chronic means it can last more than 3 months .

In some cases, the problem may last about 4-12 weeks. For such cases, it is called sub-acute. Additionally, there is a condition called recurrent sinusitis. It refers to a condition of when you have several acute sinusitis in 1 year.

What is the cause? This can vary, but viral infection is often to blame. Many people have sinus infection after a common cold event, for example. The infection can also be caused by bacteria or fungus. Sometimes environmental irritants and allergens can be also the trigger.

    What Are The Risks

Z-paks have been around long enough that the drug is known to be well tolerated by most patients, however, it does carry some risks.

According to the U.S. Food and Drug Administration, the pills can cause abnormal changes in the hearts electrical activity that may lead to a fatal heart rhythm.

A benefit versus risk analysis should be performed on patients with existing QT interval prolongation, elderly patients and patients with a history of cardiac disease, Dr. Kobic said. But, overall the risk of ventricular arrhythmia is very low and reported as less than 1% in clinical trials.

How Do Ear Infection Antibiotics Work

Ear infections are no fun for anyone.

An ear infection occurs when fluid builds up behind your eardrum in the middle part of your ear and causes swelling . This can lead to significant pain and discomfort.

Since most ear infections are caused by bacteria, its typically best to treat individual cases with antibiotics.

But antibiotic treatment isnt appropriate for every ear infection. There are a variety of factors to consider, including:

Recurring ear infections may also require a different approach.

If youre considering ear infection antibiotics for yourself or a loved one, learn more about how these medications work and how they can be both helpful and possibly harmful.

Ear infections are most prevalent in young children. Theyre often the byproducts of upper respiratory infections.

You or your child might experience other symptoms before the ear infection, including:

If an upper respiratory infection is caused by bacteria, then its possible to have an ear infection at the same time.

An ear infection occurs when bacteria gets trapped in your middle ear. Bacteria known as Hemophilus influenza and Streptococcus pneumoniae are the most common bacterial culprits.

But an ear infection may still occur if you have a viral respiratory illness. As you recover, its possible for bacteria to travel to your middle ear and become trapped, leading to a secondary infection in your ears.

    Diagnosing A Middle Ear Infection

    I Got Oral Shingles

A doctor will generally be able to diagnose a middle ear infection by considering a persons symptoms and looking for specific signs. Using an otoscope , a doctor will be able to look into the ears, throat and nose for any signs of infection. While the area around the eardrum will most likely be red and swollen, this will normally only be noticeable upon examination with an otoscope. The ear itself will generally not be red or swollen.

In some cases, especially when the condition is predicted to have taken the form of otitis media with effusion, a pneumatic otoscope may be used to confirm a diagnosis. This instrument pumps air into the middle ear and measures the resulting pressure changes. If little response is felt, this indicates the presence of fluid in the ear.

To further confirm a middle ear infection diagnosis, a doctor may order a number of further tests, including:

    Are There Alternatives To Z

The medications clarithromycin or are sometimes used as alternatives to the Z-Pak, according to Chirag Shah, MD, board-certified emergency medicine physician and co-founder of Push Health. However, these alternatives will not always work to treat the infection for which the Z-Pak was prescribed in the first place, and consultation with ones medical provider is recommended before changing medications.

Antibiotics Are More Effective Than Zpacks For Ear Infection

Recently we have been focusing on the question of whether ear infections are more difficult to treat with antibiotics than with antibiotics prescribed to people with other ailments, such as allergies. In a recent study done in the United States, second and third generation cephalosporins were found to be more effective than zpacks against a common illness that affects children otitis media . The most significant finding was that while antibiotic treatment is successful in only about a third of the cases, second and third generation cephalosporins were found to be effective in all of the cases.

This study is important because we have to recognize that there are inherent risks when it comes to taking antibiotics for an ear infection. Some studies have suggested that prolonged use of antibiotics can produce resistance to the drugs, making them less effective than zpacks. This new evidence strengthens the claim that second and third generation cephalosporins are, in fact, more effective than zpacks when it comes to treating ear infections.

Can You Treat An Ear Infection Without A Doctor

It is possible to treat some types of ear infections without help from a doctor. A middle ear infection may clear up on its own without going to the doctor. Outer ear and inner ear infections may require medical attention depending on the severity of the infection and the bodys ability to fight the infection. Common home remedies and treatments include the use of over-the-counter pain relievers, cold or warm compresses, changes in sleeping positions, and olive oil.

Theres More Nuance To Antibiotic Prescribing Than You Might Realize

    Azithromycin Tabs 250mg Z

The way doctors think about prescribing all antibioticsnot just Z-Packshas evolved since these medications were introducted.

First off, both of the experts SELF spoke to for this story emphasize that what may seem like a penicillin allergyor, more likely, what youve been told all your life is a penicillin allergymay not be a true allergy. Many people, like me, grow up being told that they had some kind of a rash or other vaguely bad reaction after getting penicillin treatment as a baby, which is very possibly true. But people grow out of these sorts of reactions, Dr. Vijayan says, and doctors now appreciate just how exceptionally rare true penicillin allergies are.

About 10 percent of patients report a penicillin allergy, according to estimates from the American Academy of Allergy, Asthma, and Immunology , but about 90 percent of them may not actually have one. So, many of the people who took Z-Packs because they thought it was their only option may have been perfectly fine just getting the penicillin instead, making them even more egregiously overprescribed.

Weve also come to understand that there are shades of gray to the conventional wisdom of taking every last dose of your antibiotic, even if you feel better long before that, Dr. Vijayan says. The truth is that, for many antibiotics that are routinely prescribed in primary care, a shorter course is totally finesometimes even betterthan a longer one.

What Other Drugs Will Affect Zithromax

Tell your doctor about all your current medicines and any you start or stop using, especially:

    • a blood thinner warfarin, Coumadin, Jantoven.

This list is not complete. Other drugs may interact with azithromycin, including prescription and over-the-counter medicines, vitamins, and herbal products. Not all possible interactions are listed in this medication guide.

CLINICAL TRIAL RESULTS SHOW SINGLE-DOSE ZITHROMAX” AS EFFECTIVE AS 10 DAYS OF AUGMENTIN IN TREATING CHILDREN’S EAR INFECTIONS

A single-dose study shows promise for effective, short-course antibiotic therapy

New York, September 14, 2000 — Pfizer Inc said today that a new clinical study shows one single dose of Zithromax is as effective as Augmentin , administered twice a day for 10 days, in treating acute otitis media in children. Results of the multicenter clinical trial were presented recently at the Infectious Diseases Society of America annual meeting in New Orleans, La.

The study found that the treatment response of children with middle ear infections who received a single dose of Zithromax was clinically equivalent to the response of those treated with a 10-day course of Augmentin . Seventy-five percent of patients in both drug groups achieved treatment success upon observation at 32 days .

Zithromax is an oral antibiotic approved for use in children and adults it achieves high and sustained levels in infected tissues making short course dosing possible.

Acute otitis media is an inflammation of the middle ear , and is often caused by bacteria. Other than colds or coughs, otitis media is the most common illness young children encounter, accounting for at least 30 million sick child visits to the doctor each year.

They think part of the problem is that patients ask for easy-to-remember names such as Z-Pack and the study found that azithromycin, the Zithromax drug that gives Z-Packs the name, is among the most commonly misused drugs.

Overall, only 52 percent of patients treated with antibiotics for sinus infections, middle ear infections and pharyngitis received the first-line treatments recommended by prescribing guidelines, Dr. David Hyun, an infectious disease specialist at the Pew Charitable Trusts, told NBC News.

This is on top of findings released earlier this year that show at least a third of people who get antibiotics dont even need them. Taken together, the two studies show a lot of bad prescribing of antibiotics, the researchers wrote in the full report published in the Journal of the American Medical Associations JAMA internal Medicine.

    Data Analysis And Synthesis

The findings of individual eligible and quality-assured studies were collated and summarized. Meta-analysis was performed using STATA software version 16 and MantelHaenszel statistical method and effect measure odds ratio was employed for data analysis, data synthesis, and creating tables . Meta-analysis was performed for studies with a similar design on the same intervention and assessing same outcome and where sufficient data were available. The results were reported in an odds ratio with 95% CI in studies. Heterogeneity I2 was analyzed from the forest plot result and publication bias was also assessed by creating a funnel plot. We used a random and fixed effects model depending on the degree of heterogeneity between studies. Results, together with the associated interpretations and conclusions were generated from narrative and quantitative synthesis, and the review was presented on a table.

    How Should Augmentin Be Taken

    For Adults:

    • The usual adult dose is one 500-mg tablet of Augmentin every 12 hours or one 250-mg tablet of Augmentin every 8 hours. For more severe infections and infections of the respiratory tract, the dose should be one 875-mg tablet of Augmentin every 12 hours or one 500-mg tablet of Augmentin every 8 hours. Adults who have difficulty swallowing may be given the 125 mg/5 mL or 250 mg/5 mL suspension in place of the 500-mg tablet. The 200 mg/5 mL suspension or the 400 mg/5 mL suspension may be used in place of the 875-mg tablet.
    • Two 250-mg tablets of Augmentin should not be substituted for one 500-mg tablet of Augmentin. Since both the 250-mg and 500-mg tablets of Augmentin contain the same amount of clavulanic acid , two 250-mg tablets are not equivalent to one 500-mg tablet of Augmentin.
    • The 250-mg tablet of Augmentin and the 250-mg chewable tablet should not be substituted for each other, as they are not interchangeable. The 250-mg tablet of Augmentin and the 250-mg chewable tablet do not contain the same amount of clavulanic acid . The 250-mg tablet of Augmentin contains 125 mg of clavulanic acid, whereas the 250-mg chewable tablet contains 62.5 mg of clavulanic acid.
