Kauffman et al. similarly reported 100% success in six patients resistant to amphotericin B . Several authors believe that the daily dosage should be raised to 600800 mg/day. In conclusion, fluconazole cannot be used as first line treatment in the setting of systemic candidiasis among neutropenic patients. It is recommended when switching initial amphotericin B therapy to oral maintenance regimen, if the patient was not previously on fluconazole prophylaxis and was not known to be colonized/infected with a less-susceptible or resistant strain. Treatment should be maintained for months, until disappearance of calcification of the lesions, especially if further antineoplastic drugs courses have to be administered.
