Identification And Preparation Of Tcm Extracts

TCM were purchased from local Chinese medicine store and were identified morphologically, histologically and chemically using standard Chinese herbal identification procedures3.

Voucher specimens including the identification and classification of all the TCMs to be tested were stored in the Hard Tissue Laboratory, University of Hong Kong. TCM extracts were prepared using standard protocols for aqueous extraction5. Briefly, 4 ml distilled water was added to 10 g of TCM powder. The mixture was boiled under stirring on a hot plate for 4 h. Distilled water was added occasionally to prevent drying. At the end of boiling, distilled water was added to make up the volume of the mixture to 4 ml. The mixture was cooled, centrifuged and filtered. This produced 2.5 g/ml of one TCM extract.

The 20 TCM drugs chosen for the study were Cortex magnoliae officinalis , Cortex phellodendri , Flos caryophylli , Flos chrysanthemi morifolii , Flos lonicerae japonicae , Fructus armeniaca mume , Fructus forsythiae suspensae , , , , Herba ephedrae , Herba portulacae oleraceae , Herba menthae haplocalycis , Pericarpium granati , Radix et rhizoma rhei , Radix gentianae , Radix isatidis , Ramulus cinnamomi cassia , Rhizoma cimicifugae and . Chlorhexidine gluconate, a common oral antiseptic, at a concentration of 0.2% was used as a positive control for all experiments.

Broth Microdilution Susceptibility Test

The TCMs, Rhizoma coptidis and Galla chinensis, which showed potent antimicrobial activity against the four tested bacteria in the agar diffusion assay screening test, were selected for minimum inhibitory concentration determination using the standard broth microdilution assay2, 6, 7.

In brief, bacteria were cultured anaerobically in HBA for 37 days at 37 °C. The bacterial cultures were harvested and suspended in an appropriate medium at a concentration of 1 × 106 cells/ml . Broth microdilution assay was performed in polystyrene, 96-well microtiter plates as previously described13. Each plate included a positive control and negative control and serial two-fold dilution of the drug. P. gingivalis suspensions were prepared in P. gingivalis broth whereas other bacterial species were prepared in brain heart infusion broth. Bacterial inoculums were prepared as described previously and 100 l of 1 × 106 cells/ml bacterial suspensions were added to each well. Subsequently, microtiter plates were incubated for 48 h at 37 °C in an anaerobic chamber. Visual inspection was used to determine the MIC. After MIC determination, 20 l of suspension from each well was inoculated in blood agar plates for 48 h for observation. The lowest drug concentration that yielded no growth was documented as minimum bactericidal concentration . For P. gingivalis all the procedures took the slow growth of the organism into account.

According to the American Academy of Periodontology, Periodontal diseases, including gingivitis and periodontitis, are serious infections that, left untreated, can lead to tooth loss. The word periodontal literally means around the tooth. Periodontal disease can affect one tooth or many teeth. It begins when the bacteria in plaque causes the gums to become inflamed.

In the mildest form of the disease, gingivitis, the gums redden, swell and bleed easily. There is usually little or no discomfort. Gingivitis is often caused by inadequate oral hygiene. Gingivitis is reversible with professional treatment and good oral home care.

Untreated gingivitis can advance to periodontitis. With time, plaque can spread and grow below the gum line. Toxins produced by the bacteria in plaque irritate the gums. The toxins stimulate a chronic inflammatory response in which the body in essence turns on itself, and the tissues and bone that support the teeth are broken down and destroyed. Gums separate from the teeth, forming pockets that become infected. As the disease progresses, the pockets deepen and more gum tissue and bone are destroyed. Often, this destructive process has very mild symptoms. Eventually, teeth can become loose and may have to be removed.

Is Extraction Necessary

Should the tooth be extracted when there is a toothache? Different doctors hold different views.

After having a few teeth extracted, it dawned on Dr. Hu that if he had not had his first extraction back then, subsequent extractions could have been avoided.

He gave an example. If an upper left back molar is extracted, it will have an impact on the corresponding upper right back molar, resulting in misalignment and tooth decay, since food will be trapped. If this tooth is removed, the corresponding tooth in the lower jaw may become longer, putting pressure on other teeth.

The pain may lead to more extractions. Therefore, Hu suggests saving every tooth with just a filling as far as possible.
