Scary Prednisone Side Effects That Have Been Reported To Us:

Sinusitis and Its Treatment | Todd Kingdom, MD, Ear, Nose and Throat | UCHealth

A.C. shared this story:

Years ago I was given prednisone in the emergency room for a severe anaphylactic reaction that affected my ability to breathe and caused massive hives. Although the treatment may have been necessary, I too had a severe psychotic reaction and when I finally went to my own doctor and had blood tests, my blood chemistry was all over the map. I had to continue the tapered dose till I was done but I wish someone had warned me of possible side effects so at least I wouldnt think I was totally crazy.

I questioned my ability to drive, slept constantly and was quite volatile. I had to take a day off from work. Knowledge is power! People should be warned about possible side effects so they have the information should prednisone side effects occur.

When people are unprepared for the psychological side effects of prednisone, they can be caught off guard. So can family, friends and co-workers. In A.C.s case, the prednisone was was essential for survival. That said, A.C. should have been alerted to possible side effects.

Ely describes what happened after a moderate dose of prednisone:

Bob describes what its like to be sleepless on steroids:

My wife had sleepless nights when on prednisone and the doctor said that she might do some odd things that she normally wouldnt do. He was right. One night she got up and tore down the wall paper in our bathroom We still get a laugh over this one.

How Is An Ear Infection Diagnosed And Treated

During a physical examination, your veterinarian will look in the ear for the presence of inflammation, redness, discharge, growths, or other findings that may indicate an ear infection. Sometimes, a cotton swab is used to collect debris from the ear. This material can be placed on a slide and examined under a microscope to determine if the infection is due to yeast, bacteria, or mites. Your veterinarian may also collect a sample of ear debris for culture and sensitivity testing, which identifies the exact organisms present and helps your veterinarian select the best antibiotic to use.

In severe cases, or if the animal is in too much pain to permit an examination of the ears, sedation may be needed to evaluate the ears, collect samples of discharge, clean the ears, and initiate treatment. With the pet sedated, the ears can be gently flushed to remove debris and facilitate better examination of the ear. Radiographs and other diagnostic tests can be performed while the pet is sedated to determine if the middle or inner ear are also involved.

If there are underlying problems such as thyroid disease or seborrhea, these must also be addressed to clear the infection and reduce the chances of recurrence.

Remedies For Inner Ear Infections

Do you need antibiotics?

If you have a bacterial inner ear infection, you may need an antibiotic. However, most inner ear infections are caused by viruses, not bacteria. Antibiotics arent effective against viral infections. For viral infections, your doctor will suggest treatment options to control your inflammation and help with your symptoms.

What treatments can help?

If you have a viral inner ear infection, your doctor may prescribe a steroid treatment to reduce inflammation or antiviral medication to attack the virus.

What are some inner ear infection home remedies?

Besides medication, there are steps you can take at home to ease the symptoms of an inner ear infection. For example:

  • Keep your head upright as much as possible. Sitting or standing can help to drain the ear.
  • Apply a warm compress to the affected ear to relieve pain.
  • Avoid smoking and drink as little alcohol as possible.
  • Reduce your stress levels, since tension can worsen symptoms.

For labyrinthitis, specifically, to combat dizziness and nausea, you can try:

  • Resting in a dark, quiet place
  • Drinking plenty of water, especially if your nausea leads to vomiting
  • Keeping your vision focused in one place instead of glancing around at your surroundings
  • Going for walks as soon as you feel you can, with someone for support until you get your balance back

Tests For Middle Ear Infection

The GP will look carefully at the inside of your childs ear using an instrument called an otoscope.

The GP might also do a tympanometry. This test measures how much your childs eardrum can move, and it can help the GP work out whether the ear is normal. Its usually a painless test that takes just a couple of minutes.

If your child has had several ear infections, or if your doctor thinks there might be a chronic infection or glue ear, the doctor might organise a hearing test. Your child can have a formal hearing test at any age.

Steroid Treatments Equally Effective Against Sudden Deafness

Injecting steroids into the middle ear works just as well as taking them orally when it comes to restoring hearing for sudden deafness patients. This finding, the result of a large clinical trial comparing the therapies, will help doctors choose the best treatment for patients with this condition.

Sudden deafness, also called sudden sensorineural hearing loss, is an emergency medical condition that affects several thousand people annually, usually between the ages of 40 and 60. It often arises without an obvious cause and occurs in one ear all at once or over a period of up to 3 days. Oral steroids, such as prednisone, are usually prescribed over the course of 2 weeks to restore hearing. There is only a 2- to 4-week window of time for treatment before hearing loss becomes permanent.

Recently, doctors have started injecting steroids directly into the middle ear a procedure called intratympanic treatment. This technique is thought to deliver more of the drug to the ear and to avoid some of the side effects that can come along with oral steroids. The side effects of oral therapy can be mild, like weight gain, mood changes and sleep disruption, or more serious, like high blood pressure and elevated blood sugar. Side effects of injected steroids are usually local, such as ear infection and vertigo. However, up until now, no study had compared the 2 treatments to see whether direct injection worked as well as oral steroids.

Harm Of Gcs In Children

How To Prevent Ear Infections In Dogs

There is limited knowledge of risks of using systemic GCS in pediatric CRS or ARS compared to pediatric asthma. As an example, the Childhood Asthma Management Program trial followed the annual bone mineral accretion of 877 children with mild-to-moderate asthma . Oral GCS bursts produced a dosage-dependent reduction in bone mineral accretion and an increase in risk for osteopenia for 0, 14, and 5 courses, respectively, in boys. The authors conclude that multiple oral GCS bursts over a period of years can produce a dosage-dependent reduction in bone mineral accretion and increased risk for osteopenia in children with asthma. 780 children with asthma were followed for a mean of 4.3 years and it was shown that boys with lower vitamin D levels are significantly more susceptible to the negative effects of GCS on bone mineral accretion over time . Regarding studies investigating GCS AEs in upper airway disease, the trial from Ozturk also looked at self-reported AEs during the 15-day course of oral MP . In this trial no clinically significant AEs were reported. At the end of the treatment, the mean weight change did not differ statistically significantly between the groups. No data of monitored AEs, nor that of long-term outcomes, nor that of bacterial culture were available in this study.

How To Use Prednisone

If you have been prescribed oral Prednisone, you should take it with milk or food to prevent bloating or stomach upset. Take oral Prednisone with a glass of water, unless your doctor says otherwise. If you are taking Prednisone syrup, you should use a special measuring spoon to carefully measure its dose. Using a kitchen spoon may not measure the correct amount. If you need to take only one dose a day, we recommend taking it in the morning, since it may disturb sleep.

Take your Prednisone dosage exactly as instructed by your physician. Follow the schedule and dosage amount carefully. The dosage of the medicine and the length of your treatment largely depends on your: individual medical condition, the severity of your disease and your response to the treatment.

You should never stop taking Prednisone unless your doctor advises you to do so. If you suddenly stop taking this medicine without medical advice, in some cases, your conditions may worsen. You may also start experiencing symptoms such as: weakness, nausea, weight loss, headaches, dizziness, tiredness, muscle pain etc. In order to prevent such symptoms, your doctor will advise you to reduce the dosage slowly. If you develop any new symptoms during this time, you should let your doctor know immediately.

Cautions With Other Medicines

There are many medicines that interfere with the way prednisolone works, or that increase the risk of side effects. Its very important you check a medicine is safe to take with prednisolone before you start taking it.

This includes some medicines that you buy over the counter like aspirin and anti-inflammatory painkillers such as ibuprofen.

Treatment For Otitis Media With Effusion

Otitis media with effusion is fluid behind the middle ear . It usually resolves on its own without treatment, especially when it follows an acute ear infection. Antibiotics are not helpful for most cases of OME.

Clinical practice guidelines for OME recommend the following treatments:

  • Watchful Waiting for OME. The child is typically monitored for the first 3 months. If OME lasts longer than 3 months, a hearing test should be conducted. Even if OME lasts for longer than 3 months, the condition generally resolves on its own without any long-term effects on language or development. The doctor will re-evaluate the child at periodic intervals to determine if there is risk for hearing loss.
  • Drug Treatment. Antibiotics, decongestants, antihistamines, and corticosteroids do not help and are not recommended for routine management of OME. Antibiotic ear drops are helpful for treating ear infections that may occur in children with tympanostomy tubes. Topical antibiotics work better than oral antibiotics for treating the discharge that can occur with this type of infection.
  • Surgery. Ear tube insertion may be recommended when fluid builds up behind your child’s eardrum and does not go away after 3 months or longer. Fluid buildup may cause some hearing loss while it is present. However, most children do not have long-term damage to their hearing or their ability to speak even when the fluid remains for many months.

Tonsillectomy is not recommended for OME treatment.

Treatment For Acute Otitis Media

Most cases of AOM clear up on their own within a week and do not require antibiotic treatment. Doctors often recommend a “watchful waiting” period for the first 48 to 72 hours after symptoms appear, to see if ear pain and other symptoms resolve on their own.

For antibiotic treatment, the latest recommendations are:

  • Children younger than 6 months of age should receive immediate antibiotic treatment.
  • Children 6 months or older should be treated for pain within the first 24 hours with either acetaminophen or ibuprofen . Pain relievers — not antibiotics — are the main drugs used for AOM treatment.
  • For children, aged 6 months to 2 years old, antibiotic treatment is recommended for either severe symptoms or for non-severe symptoms that have not improved within 48 to 72 hours. Severe AOM symptoms include moderate to severe pain and a fever of at least 102.2°F .
  • For children older than 2 years, and those with mild symptoms or infection only in one ear, watchful waiting is recommended.
  • Preventive antibiotics are not recommended for recurrent acute otitis media.

Ear tube insertion is an option for children who have had at least 3 occurrences of AOM in 6 months or 4 episodes in a year. However, newer guidelines strongly advise that tympanostomy tube surgery should be used only for children who have middle-ear effusion and not for children with frequent AOM infections.

Treating Outer Ear Infections

Outer ear infections are sometimes called swimmer’s ear, or otitis externa. This is an infection of the ear canal, the portion of the ear that leads from the outside and stops at the eardrum. The opening of this part of the ear is external and visible.

Outer ear infections are called swimmer’s ear because they can sometimes be caused by swimming or bathing in contaminated water.

This is not the only way to get an outer ear infection, however. Bacteria, viruses, or fungi can be introduced to the outer ear in many ways, especially through broken skin, and usually result in an infection when a moist environment aids their growth.

Symptoms of an outer ear infection can include:

  • Pain in the ear
  • Redness and irritation inside the ear canal
  • Itchy ear canal
  • Flaky or peeling skin

More severe infections can lead to swelling of the ear canal, which may lead to muffled hearing, a fever, or ear drainage that looks like there is pus in it.

An outer ear infection can be diagnosed through an examination of the ear canal with an otoscope .

How To Use Prednisolone Ear Drops

  • Lie down, or tilt your head to the side, so that the affected ear is facing upwards.
  • Gently pull your earlobe downwards to straighten the ear canal.
  • Hold the bottle upside down near to your ear and apply enough pressure to release two or three drops into your ear.
  • Keep your ear facing upwards for a few minutes to allow the solution to come into contact with the affected area.
  • Repeat the process in your other ear if you have been asked to use the drops in both ears.
