More than ever before, there is a prominent role for shared decision-making between patients and clinicians when managing adult sinusitisespecially in deciding whether to use antibiotics for acute bacterial sinusitis or to instead try watchful waiting to see if a patient can fight the infection on his or her own.

Ah, sinus infections. The New England Journal of Medicine published a clinical practice review of acute sinus infections in adults, that is, sinus infections of up to four weeks. The need for an updated review was likely spurred by the disconcerting fact that while the vast majority of acute sinus infections will improve or even clear on their own without antibiotics within one to two weeks, most end up being treated with antibiotics.
It is this discrepancy that has clinical researchers and public health folks jumping up and down in alarm, because more unnecessary prescriptions for antibiotics mean more side effects and higher bacterial resistance rates. But on the other hand, while 85% of sinus infections improve or clear on their own, theres the 15% that do not. Potential complications are rare, but serious, and include brain infections, even abscesses.
What Are The Dosages Of Metronidazole
Dosage of Metronidazole:
- Loading dose: 15 mg/kg intravenously not to exceed 4 g/day

- 1 g orally every 6-8 hours for 3 doses
- 15 mg/kg IV over 30-60 min complete approximately 1 hour before surgery may administer 7.5 mg/kg IV over 30-60 min at 6 and 12 hours after initial dose for maintenance discontinue within 12 hours after surgery
- Adult: 250 mg orally every 8 hours for 7 days alternatively, 375 mg orally every 12 hours for 7 days
- 2 g orally once daily single dose alternatively, 1g orally every 12 hours for 2 doses
- Pediatric: less than 45 kg body weight: 15 mg/kg/day IV/oral divided every 8 hours for 7 days not to exceed 2 g/day
- Adult: 500-750 mg orally every 8 hours for 5-10 days
- Pediatric: 35-50 mg/kg orally divided once every 8 hours for 10 days

When To Consider Antibiotics For Sinus Infections
