A female patient, after suppression of antiretroviral therapy, has absolute control of the replication of HIV, maintained for more than 15 years, with an undetectable viral load and without taking medication to combat the virus.

Rash Related To Medication

Reconstitution of the CD4+ T-cell compartment after CCR532/32 SCT was associated with an expansion of activated memory cells , the preferential targets of productive HIV infection. Donor-derived CD4+ T cells are naturally resistant to CCR5-tropic HIV infection because of the lack of CCR5 surface expression. We were interested in whether recovered CCR532/32 CD4+ T cells might also exhibit reduced CXCR4 surface availability. Therefore, we analyzed fresh whole blood cells and MMC for CXCR4 surface expression on CD4+ T cells in comparison with cells obtained from CCR5 wild-type patients. As shown in A, both the frequency of CXCR4-expressing cells within memory CD4+ T cells as well as CXCR4 surface expression density at the single cell level were comparable with those of CCR5 wild-type control patients . This was also observed for the peripheral naive CD4+ T-cell compartment .

Susceptibility of recovered CD4+ T cells in the central as well as the mucosal immune system to productive HIV infection was studied by ex vivo infections of PBMC and MMC obtained after CCR532/32 SCT. As shown in , cells from both compartments were susceptible to productive infection by X4 HIV. Consistent with our previous observation, virus production of the PBMC-propagated X4 HIV strain was greater in peripheral than in mucosal CD4+ T cells. As expected, because of the lack of CCR5 surface expression on donor-derived cells, both peripheral and mucosal CD4+ T cells were resistant to R5 HIV infection.

Cell Preparation And Activation
Peripheral blood mononuclear cells were isolated from heparinized venous blood by standard Ficoll gradient centrifugation, and mucosal mononuclear cells were isolated from colon biopsy specimens by collagenease type II digestion. Cells were either immediately used for subsequent analysis or cryoconserved until HIV susceptibility assays. For some experiments, PBMCs were activated for 2 days with 3 g/mL of phythemagglutinin and 50 U/mL of recombinant interleukin-2 in RPMI 1640 + GlutaMAX cell culture medium containing 10% heat-inactivated fetal calf serum , 100 U/mL of penicillin, and 100 g/mL streptomycin before flow cytometric analysis.
There Have Been Two Cases Of People Cured Of Hiv
There are two cases where researchers cured HIV entirely, both as part of the sterilising approach.
The first in was Timothy Brown , who received chemotherapy and a bone marrow transplant as part of his leukemia treatment in 2007. The transplant was from a donor who had a natural resistance to HIV, and following Browns transplant, he appeared to be free of HIV.
Following this, doctors replicated this result on another patient, Adam Castillejo, or the London Patient, where following his transplant, became HIV-free. As of 2020, 30 months after stopping treatment, Adam was still HIV-free.

The best ways to keep from spreading HIV to others are many of the same ways you use to protect yourself:
- Let sexual partners and anyone you inject drugs with know that you have HIV.
- Follow your treatment plan and dont miss medications. If you have an undetectable viral load, you greatly reduce the risk of transmitting HIV through sex.
- Talk to your sexual partner about taking PrEP.

A Unique Case: 15 Years With Absolute Control Over Replication Of The Virus
The study presented at the 2022 AIDS Conference describes the immune mechanisms of a female post-treatment controller that confer complete control of HIV replication over more than 15 years. The patient was diagnosed in the acute phase of HIV infection and was enrolled in a clinical trial with antiretroviral treatment for 9 months and different immune-modulating interventions with ciclosporin A, an immunosuppressant agent. The patient did not have genetic factors associated with control of HIV, she was not an elite controller of the disease, and furthermore, she presented severe acute-phase infection, something that is uncommon in post-treatment controllers“, said Josep M. Miró.
Furthermore, it has been shown that the patient is not infected with defective viruses, i.e., her virus was viable because it was possible to isolate it and culture it in the laboratory. Over these years, we have found a pronounced and gradual drop in the number of viruses in the reservoir, which suggests control by the immune response“, explained Sonsoles Sánchez-Palomino.
The researchers saw that the patients blood cells were highly resistant to infection by the HIV virus in in vitro cultures, but her purified CD4+ T cells were susceptible to infection to HIV. This suggests that other populations of blood cells were blocking the infection and may contribute to controlling the HIV.
