Diagnostic testing is recommended for chronic sinusitis, either computed tomography of the sinuses or a nasal endoscopy, but there is disagreement about which should be the first-line diagnostic tool.
Raymond G. Slavin, MACP, a professor of internal medicine and molecular microbiology and immunology at St. Louis University School of Medicine in Missouri, said that a diagnosis can be made without either of these tests, but if there is any question, a limited CT scan can be helpful. It is considered the gold standard for diagnosis.
A CT scan can be most useful if the symptoms are vague, the physical findings are equivocal, or there has been a poor response to the initial management, according to 2005 guidelines from the American Academy of Allergy, Asthma and Immunology . One study cited in the guidelines found, however, that more than 50% of patients with a strong history of chronic sinusitis had a normal CT scan.
According to 2007 guidelines from the American Academy of Otolaryngology-Head and Neck Surgery Foundation , patient symptoms and quality of life do not necessarily correlate with the extent of disease seen on CT, but the CT does offer an objective method for monitoring chronic disease, and it can quantify the extent of the inflammation.

What Is Bacterial Sinusitis
Bacterial sinusitis is a bacterial infection of the paranasal sinuses, the hollow spaces in the bones of the face around the nose. Sinusitis is a very common problem, affecting approximately one in every eight American adults annually. Almost 30 million people seek medical help for sinusitis in the USA every year.
Bacterial sinusitis involves inflammation of the mucous membrane lining of the sinuses, and is caused by a bacterial infection. There are four pairs of paranasal sinuses which are located below, above, between and behind the eye sockets â the maxillary sinuses, frontal sinuses, ethmoid sinuses and sphenoidal sinuses. The maxillary sinuses are the largest of these, and are most often affected by sinusitis. However, sinusitis can affect any or all of the paranasal sinuses. Because sinusitis often occurs along with rhinitis, inflammation of the mucous membrane in the nose, it is sometimes known as rhinosinusitis.
Bacterial sinusitis often follows a viral infection, such as a cold or the flu. Viral infections can cause the mucosal lining of the sinuses to swell. In healthy sinuses, mucus drains into the nasal cavity via small holes, known as ostia. Swelling of the mucus membranes in the nose or sinus cavities can block these holes, preventing mucus from draining out of the sinuses. Bacteria colonize the undrained mucus, which can lead to bacterial sinusitis.
Treatment For Sinus Infection And Dizziness
Treatment varies based upon the diagnosed cause. In some cases, the sinusitis and symptoms will resolve without treatment, however only a licensed physician is capable of making this diagnosis. A doctor should be contacted if symptoms do not clear up within a few days, or if symptoms get worse. Sinusitis that lasts for longer than 8 weeks may be a sign of another underlying illness.
Treatment is given after sinusitis is diagnosed after a physical exam. An X-ray or CT scan may be used for diagnostic purposes in more severe cases. Sinusitis that is bacterial in origin may be treated by antibiotics. There are also over the counter medications like decongestants and anti-inflammatory medications that are typically recommended by a physician for treatment. Expectorants can thin the mucus and help improve sinus drainage. A pharmacist or physician should be consulted about the proper usage of over the counter medications and prescriptions.
Home therapies like hot showers and steam treatments can be effective complements to other treatments for sinusitis. Steam inhaled three or four times per day using a steam vaporizer about ten minutes each time can be beneficial. Mentholated medications like Vicks Vapo-Rub are also beneficial for opening the sinuses and promoting drainage.
