Fluconazole, a drug commonly used to treat vaginal yeast infections, can lead to a greater chance of miscarriage if taken orally during pregnancy, according to a new study by Université de Montréal researchers.

While topical azoles applied as creams or ointments are the usual go-to treatment for pregnant women with fungal infections, oral fluconazole ingested in tablet form is frequently prescribed as well.

A team led by UdeM pharmacy professor Anick Bérard looked at data on 441,949 pregnancies listed in the Quebec Pregnancy Cohort between 1998 and 2015, and linked them to filled prescriptions in the Quebec Prescription Drug Insurance database.

In their study, today in the Canadian Medical Association Journal, the researchers found that taking oral fluconazole was linked to adverse outcomes in 2 to 3 per cent of cases.

Our study shows that taking any dose of oral fluconazole while pregnant may be associated with a higher chance of miscarriage, although the risk remains small, said Bérard, who holds UdeM’s Research Chair on Medications, Pregnancy and Lactation.

Safe Prescribing Is Key

The study re-emphasizes safe prescribing practices in pregnancy,” commented Drs. Vanessa Paquette and Chelsea Elwood, of Vancouver’s British Columbia Womens Hospital and Health Centre, writing in the CMAJ.

Doctors should take the right steps by first confirming the correct diagnosis “and then choosing the safest medication with the largest body of data in pregnancy at the lowest appropriate doses,” they advised.

Women shouldn’t be unduly worried, however. “The risk of complications is, on the whole, very low,” said Bérard, who runs the Medications and Pregnancy unit at CHU Sainte-Justine children’s hospital.

“Is it worse to leave a yeast infection untreated? Absolutely. But the patient and doctor should at least have a discussion about the appropriate dose. In principle, less is always safer than more. That should be the norm.”

Women can also ask their pharmacists for alternative medications, some available over the counter, to treat their yeast infection, added Bérard, who is now looking at extending the study to patient cohorts across Canada.

How Is Candidiasis Treated

Most of the time, vulvovaginal candidiasis is easy to treat with an antifungal cream or suppository. The medicine should relieve your symptoms within seven days. However, during pregnancy, you should see your doctor before starting treatment. They can confirm that you actually have a yeast infection and ensure you get a treatment thats safe to use during pregnancy.

Both oral and topical antifungal drugs are used to treat yeast infections in nonpregnant women. However, oral drugs may not be safe to use during pregnancy.     Theres no guaranteed method of prevention, but whether youre pregnant or not, here are some ways to decrease your chances of getting a yeast infection. In general, you want to try to let the vaginal area breathe and minimize the amount of moisture down there, says Phillips.

    Edit your underwear drawer. Make sure to choose underwear and tights with cotton crotches, and avoid wearing thongs or anything thats too tight. You should also remove underwear before going to bed.

    Change out of damp stuff pronto. Dont wear wet swimsuits or gym clothes for longer than needed, and wash them each time you wear them.

    With bottoms, go for a flowy look. If youre opting for pants, keep them loose-fitting and breathable.

    Stock up on probiotic foods. For example, eating yogurt containing live Lactobacillus acidophilus cultures may help keep your vaginal pH in check.

    Skip scented products. Dont take baths with scented products and avoid douching, vaginal sprays and sanitary pads with fragrance.

    Get bathroom hygiene right. It bears repeating: Always wipe from front to back after using the restroom.

    Vaginal candidiasis, commonly called “yeast infection,” is relatively common during pregnancy, with an estimated prevalence of 10%-75%. The patient usually presents with vulvar pruritus, burning, soreness, and irritation, with occasional dysuria.

    Pregnancy causes increased levels of progesterone and estrogen. Progesterone suppresses the ability of neutrophils to combat Candida, and estrogen disrupts the integrity of vaginal epithelial cells against such pathogens as Candida and decreases immunoglobulins in vaginal secretions. These issues, which continue throughout pregnancy, lend themselves to multiple recurrences of infection.

    Treatment is directed at symptom relief. Topical imidazoles are most commonly recommended. Although both miconazole and clotrimazole are available without a prescription, pregnant patients should never self-medicate and should only use these products under the direction of a healthcare provider.

    Data in pregnancy for other topical antifungal agents are limited, making miconazole and clotrimazole the preferred topical agents in pregnancy.

    • If you are experiencing pain in the lower abdomen coupled with vaginal discharge and fever, this may be a sign of PID, and treatment will be necessary.
    • If you are pregnant and begin to experience signs and symptoms of a UTI, or urinary tract infection, or other vaginal infection
    • Unusual vaginal itching
    • Unusual vaginal discharge, especially if a foul odor accompanies it
    • Pain occurring with urination or penetration
    • Symptoms persist after initial treatment and medication
    • If symptoms recur within a couple of months from the first infection
